  • While we are contracted with many insurance providers, there may be some we are out of network with. We are currently contracted with Dean Health Plan, Quartz, GHC, Medicare, Medicaid, Alliance, WPS, HealthEOS, MultiPlan amongst others. If you do not see your insurance listed, please contact our office at 608-257-4256.

  • Yes, your physician’s office can fax or email a prescription to us. Once that is received, we will call you to set up an appointment. We are unable to schedule appointments without a prescription on file. If you just left your physician’s office and have it with you, stop on in! If we are unable to see you at that moment, we can at least get you registered and scheduled.

  • You will need your insurance cards and photo ID for your first appointment.

  • Depending on your insurance requirements, the time from evaluation to delivery can take 4 to 6 weeks.

  • Your out of pocket amount will depend on the type of device you are receiving and your insurance coverage. Our Billing Department will contact you prior to fabrication of your device if your out of pocket is over $100.00.

  • Yes! If you have ANY insurance change at any point in this process, it is imperative that you contact our office immediately. This can possibly impact the timeline for delivery, coverage and out of pocket expenses.

  • We do not bill your insurance until you have received your device. The date of service billed is the date of your definitive fitting.

  • Yes. Most devices are under a 90 day warranty that begins the date of delivery. This warranty does not cover changes in your physical weight, condition, negligence, alterations or physiological changes that may occur.

    If your device requires repairs or adjustments, please contact our office as soon as possible for the next available appointment. Outside of the warranty period for your device, your insurance may be billed.

  • No. Your device was custom fabricated for your individual needs and fit. In addition, hygiene concerns prevent your device from being returned.
